We also provide support for return-to-work and school documentation when appropriate, ensuring care that is both clinically sound and practical for everyday life.

Triage & Clinical Guidance Visit
Best for: “I’m not sure what I need yet.”
What it includes:
Brief symptom review
Clinical triage and risk assessment
Guidance on next steps (home care vs. in-person evaluation)
Care recommendations or referral guidance
Safety net instructions
What qualifies:
Mild symptoms or early illness
Uncertainty about whether treatment is needed
“Do I need to be seen?” concerns
Single symptom or early-stage concerns
Not appropriate for:
Prescription-only requests
Moderate/severe illness requiring treatment
Multiple or worsening symptoms

Core Urgent Care Visit
Best for: “I know I need treatment.”
What it includes:
Full virtual evaluation
Diagnosis and treatment plan
Prescriptions when appropriate
Work/school excuse notes (if clinically appropriate)
Patient education + symptom management plan
Common conditions:
Cold, flu/COVID, sinus infections
UTIs
GI illness (nausea, vomiting, diarrhea)
Viral infections
Mild respiratory infections
Gynecological infections (BV, yeast infection)
What qualifies:
1-7 days of acute symptoms
One primary issue (or clearly related symptoms)
Mild to moderate illness
Needs diagnosis and/or medication
Not appropriate for:
Severe chest pain or shortness of breath
Complex chronic disease management
Emergency-level symptoms

Acute Refill Support
Best for: “I need a refill for a short-term or previously prescribed medication.”
What it includes:
Clinical review of current symptoms/need
Medication safety check and appropriateness assessment
Short-term refill when clinically indicated
Guidance on follow-up with primary care provider
Referral recommendation for ongoing management
What qualifies:
Previously diagnosed condition requiring short-term continuation
Lost or expired medication that was previously prescribed
Acute condition previously treated by a provider (including this practice)
Stable, uncomplicated need for continuation of therapy
Not appropriate for:
New diagnosis requiring full workup
Controlled substances (pain medications, ADHD, phentermine, etc.)
Complex or worsening symptoms needing full evaluation

Work/School Note - When Clinically Appropriate
Best for: “I was sick and need documentation.”
What it includes:
Brief clinical review of symptoms and timeline
Determination of whether documentation is appropriate
Work/school excuse note if medically justified
Limited clinical guidance if needed
What qualifies:
Recent acute illness (within appropriate timeframe)
Symptoms consistent with mild/moderate viral or acute illness
Patient treated (at-home) and clinically fit to return to work/school
Not appropriate for:
Requests without illness or illness or clinical justification
Back-dated (beyond practice protocol or speculative documentation)
Chronic, absence documentation
Work-specific forms such as FMLA
